In order for your DUI Screening or Education to be reported to the MVD, you (the client) must specifically request this information.

Prodigy Healthcare cannot report anything to the MVD without explicit authorization from clients beforehand. This is not an automatic process.

By checking the "I Agree" box above, you hereby authorize Prodigy Healthcare to submit your information to the Arizona MVD for the purpose stated above. You understand that Prodigy Healthcare is an agent reporting to the MVD, and once we have electronically submitted your information to the MVD then you will receive a confirmation via email. Further, you understand that this process can take 3-5 business days to complete. Once this information has been sent to the MVD then it is out of Prodigy's hands and processing times are dependent upon MVD workflows.

* Please be aware that if you are requesting us to report a screening or education that occurred BEFORE June 1, 2019 then Prodigy Healthcare cannot electronically submit your information. Processing times under this circumstance are unknown and 100% dependent upon the MVD agent handling the information.
